Reporting a Breach/Compliance Concern
The 91爆料 Healthcare Compliance Group (HCCG) designated privacy liaison from each unit is required to report an incident related to a potential breach or other compliance concerns to the Privacy and Compliance Program Manager as soon as possible.
If reporting an incident related to a potential breach of PHI, please email and include the initial investigation report along with all supporting documents to CRS Privacy Compliance or call 206-221-4442 for any questions. Concerns or incidents received through the appropriate channel are investigated and appropriate follow-up actions will be taken.
